The Susan J. Rheem Adult Day Center (SJRC), located in Prescott Valley, AZ is a nationally recognized, and licensed AZ Department of Health Services adult day health program. SJRC was founded in 1982. SJRC mission is “Quality Care. Quality Service. Quality of Life.” SJRC serves over 100 participants annually living with illnesses and disabilities related to but not limited to Alzheimer’s and dementia, Parkinson’s disease, strokes, aging, depression and isolation.
Participants enjoy programs from a highly trained team of personnel members: nursing care; personal care; entertainment and activities that are spiritual, physical, cognitive, emotional, and creative and in nature in therapeutic walking gardens; outings into the community; home cooked meals by on-site chef; care management; and transportation to and from the participants’ home. Participants are well cared for during their time at the day center having fun and forming friendships. Caregivers receive respite, rejuvenation and peace of mind. SJRC transportation service area includes Prescott, Prescott Valley, Chino Valley, Paulden, Dewey, Humboldt, Cordes Lakes and Spring Valley. SJRC maintains contracts with the Veteran’s Administration, NACOG-AAA, AHCCCS, AZ Caregiver Coalitions, the Alzheimer’s Association, the Dept. of Education CACFP program, and the AZ Dept. of Transportation.
